This may be possible if:
- the student has not yet registered onto the assessment portal
- the request is made within 6 months of the initial enrolment, and
- all the training materials are unused and intact so can be passed on to another member of staff.
Please call the CIG helpline for help. You will need to provide:
- Name/email/employee number of member of staff who has left
- Name/email/employee number of new candidate and email address
- New candidate date of birth and ethnicity, plus whether the candidate is registered disabled
- Pharmacy name and address, postcode, phone number and email (if different)
- Supervisor name/GPhC number (if different)
